2024-2025 Health Examination Requirements with New Grade Levels
New York State law requires that all NEW entrants and students in Pre-K, K, 1st, 3rd, 5th, 7th, 9th & 11th grade, have a health examination by a New York State licensed provider. This examination must be provided to school within 30 days of entry into school or the grade levels indicated above.
Acceptable health certificates may be dated anytime within the 12 months prior to the start of the 2024-2025 School Year. If a copy of the health examination is not on file within 30 days, you will be notified by the health office. We recommend that this examination be made by your family health care provider who knows your child best. During the doctor's visit, please check with your child’s physician if your child is due for any mandatory grade-level immunizations. PHYSICAL/ HEALTH EXAMINATIONS:
Along with new students and grades Pre-K, K, 1, 3, 5, 7, 9 and 11 grade, physical exams are also required for athletic participants and for working papers. A physical exam will be done by our school physician in the health office unless you have contacted the office and stated your preference to have your child seen by their own physician. If such is the case, please have the physician fax the completed form to (716) 699-5424. Click below for: PHYSICAL EXAMINATION FORM.All ECS students will have their vision and hearing screened in the Health Office yearly. Middle/high school boys may be examined for their Tanner stage for sports. You may wish to discuss this with your son, so he will know what to expect during the exam. Please know that all information is confidential and will be transferred onto the student’s permanent health record. Girls in Grades 5, 7 and boys in 9th grade are to have a scoliosis examination. If a student's private doctor did not perform the exam on their yearly physical, the school nurse will check for curvature of the spinal column. Parents would be notified of any screening concern.
As part of a required health examination, a student is to be weighed and his/her height is measured. These numbers are used to figure out the student’s body mass index or “BMI”. The BMI helps the doctor or nurse know if the student’s weight is in a healthy range or is too high or too low. These figures are included as part of the student’s school health record. Every other year, school districts are required by New York State to submit information about our student’s weight status groups. Only summary information is sent. No names and no information about individual students are sent. You may choose to have your child’s information excluded from this survey report with a completed form that is submitted to the nurse.
