• Important Sports Clearance Information

Outlines what every student athlete needs to do to be medically cleared.  Mr. Schwartz has asked that Mrs. Hager make sure every athlete has had and passed an ImPact Baseline test for concussion before they are cleared as well.  This means for Mrs. Hager to clear an athlete to practice or play they must have had:

A physical in the past year.

The Interval History Form filled out and signed by a parent (no sooner than 30 days before the start of the season).

A urinalysis to test for protein or glucose.

An ImPact test (performed here at school).

Other health forms turned in if they have a medical issue such as asthma, diabetes, seizures, or anaphylactic reactions requiring an epinehprine delivery device  (EpiPen or Auvi-Q).

  • Athletic Placement Process

The Athletic Placement Process is for 7th and 8th grade students who want to play a JV or Varsity sport.  The new state regulations state that the student must go thorugh this new process before being able to play at the higher level.  Please see Mr. Schwartz to express your interest and to receive a form to fill out.  Once the form is filled out and the parent permission slip has been signed, then the athlete must be cleared by the school's Medical Director (the athlete may be seen by their primary care physician originally, but all data must be sent to the Medical Director for FINAL CLEARANCE).  Once the athlete has been cleared, they may then start the physical fitness testing with the gym teacher.  The student must be cleared and pass the physical fitness test before they can participate in JV or Varsity practice or games.  Please be sure to see Mr. Schwartz as early as possible if you are a 7th or 8th grader and want to play JV or Varsity because this can be a long process.
