Health Information
Annual Screening:
All ECS students will have their vision and hearing screened in the Health Office yearly. Girls in grades 5 & 7 and boys in grade 9 are required to have a scoliosis examination. If a student's private doctor did not perform the exam on their yearly physical, the school nurse will check for curvature of the spinal column. Parents would be notified of any screening concern.
Medications:
If a student will need to take or carry medication at school, a medication form must be completed by the parent and the doctor. This must be done for each medication. This includes over the counter medications (e.g. Tyleonl, motrin. etc.). If a student needs to independently carry a medication, such as an inhaler or epi pen, an additional form must be completed.
These forms are only valid for 1 year.
Physicians should also provide emergency care plans when applicable. (e.g. asthma, seizures, allergies, etc.). Please see the below forms:

Sports:
Athletes must have a physical exam within 1 year of the start of the season. Interval health history information will be completed for each sport through the Arbiter athletic registration. See athletics for more information.
Concussion Protocol:
Athletes must take an impact test prior to participation in sports. This is usually given by the athletic director.
If a student is diagnosed with a concussion, they will not be able to participate until they are cleared by their physician. Once cleared they will begin return to play protocol.

Student Accident Insurance:
ECS accident insurance is an excess coverage only policy to supplement existing family medical insurance. The student's family is responsible for first submitting accident claims for payment under their "primary" insurance coverage plans. All claims will be reviewed. It is not guaranteed that medical reimbursement will be 100%.
Reporting Procedure:
- Report all injuries to coach/teacher and school nurse immediately. The school nurse will complete the injury/incident report.
- The school nurse will complete part "A" of the student accident claim form and mail it to the injured child's parents.
- The parent completes part "B" of the claim form and follows the claim instructions for submitting claims.
Physical Exams:
New York State law requires that all new entrants and students in Pre-K, K, 1st, 3rd, 5th, 7th, 9th and 11th grade, have a health examination by a New York State licensed provider. This examination must be provided to school within 30 days of entry into school or the grade levels indicated above.
Acceptable health certificates may be dated anytime within the 12 months prior to the start of the school year.
If a copy of the health examination is not on file within 30 days, you will be notified by the health office. We recommend that this examination be made by your family health care provider who knows your child best. If no physical is provided after notification, a physical exam will be done by our school physician in the health office.

As part of a required health examination, a student is to be weighed, and his/her height is measured. These numbers are used to figure out the student’s body mass index or “BMI”. The BMI helps the doctor or nurse to know if the student’s weight is in a healthy range or too high or too low. These figures are included as part of the student’s school health record. Every other year, school districts are required by New York State to submit information about our student's weight status groups. Only summary information is sent. No names or information about individual students are sent. You may choose to have your child’s information excluded from this survey report by notifying the health office.
Also, suggested by the NYS Dept. of Health is to have your child’s dentist fill out a dental health certificate at their next visit.
